Whether you want to heal alone or alongside others who can relate to you, our sessions are geared toward connecting all people to their true inner selves. Services provided by Prickly Pear are ultimately offered at your own pace and with the end goal to help you tackle all of your obstacles. Requesting Your Health Care Records
You have the right to request a copy of your mental health records
We are required to retain your records for 7 years, or 5 years after a former minor client has turned 18.
To request your records, please contact Dr. Hurtado at [email protected].
Records will be provided in accordance with Texas law and applicable privacy regulations.
Please allow for 15 business days for us to complete your request. Administrative charges may apply.
